Tumors in the heart are relatively uncommon with a prevalence of 1 in million population, of these the most common is a LA Myxoma with a prevalence of 0.5 in a million population. The common site of origin of this tumor is the Left Atrium, a chamber which receives oxygenated blood from the lungs; both sporadic and familial occurrences are known. The tumor has a tendency to grow to a large size with a likely chance to obstruct the valve between left atrium and left ventricle. When it obstructs this valve partially, patients develop symptoms like breathlessness on exertion, but when it obstructs completely, total blood flow to the body and the brain gets interrupted. The patient presents with sudden loss of consciousness or sudden paralysis.
The cardiac surgical team of KIMS Hospital led by Dr. Sameer Diwale, Dr. Dayasagar Rao, Dr. Sandeep. A. J. And Dr. Murali Mohan, operated on a highly complex case recently. This was a 26 yrs old lady, who was 24 weeks pregnant. She presented with sudden onset of breathlessness and bleeding from the birth canal. Patient underwent an echo cardio graphic evaluation, which demonstrated a 5 x 5 cm tumor in the left atrium partially obstructing the valve between Left Atrium and Left Ventricle. The situation demanded an urgent surgical procedure on the heart to remove the tumor. This surgical procedure requires the patient to be connected to a heart lung machine which takes over the function of the heart. The heart is stopped, opened and the tumor removed. Then the patient is disconnected from the heart-lung machine. This surgical procedure poses a risk both to the mother and the unborn baby. It was vital to save both the mother and the unborn baby.
The surgery which required expert anesthetic management was then performed quickly and without any blood loss. Following the procedure involving the removal of the tumor, both the mother and the unborn baby were in a healthe condition. The patient was sent home on the 5th Post operative day. The surgery was performed by the expert anesthetic and cardiac surgical team at KIMS, Secunderabad who are also performing coronary artery bypass surgery, vascular surgery and pediatric cardiac surgery.
