ASD DEVICE CLOSURE

    CASE SUMMARY

45 years old lady from from Jabalpur diagnosed to have heart disease (large ASD) in 2003 when evaluated for Dysnea. Patient was advised to go for ASD surgical closure in view of large defect with floppy rims. Symptomatic with dyspnea class II and easy fatigua-bility. She was on regular Eltroxin in view of hypothyroidism. She is known hypertensive ,was on m antihypertensives.Referred to KIMS hospital for further evaluation and man-agement. Evaluated at KIMS OPD , advised TEE to decide for device suitability. TEE evaluation was done at KIMS - found to have large ASD with thin floppy inferoposterior rim, borderline SVC rim and absent retroaortic rim with normal PA pressure. Advised bal-loon assisted ASD device closure.ECG showed sinus rhythm, right axis deviation and normal PR interval. Chest X ray showed cardiomegaly with increased pulmonary vascularity. Examination revealed weight of 65 kg,normal S1,wide and fixed split S2,3/6 ESM at LUSB.

Taken for ASD device closure on 30.03.16. Underwent Coronary angiogram that re-vealed normal coronaries. Successful ASD device closure was performed using 42 Lifetech Septal occluder with Balloon assistance using 24 mm sizing balloon. Snare assisted delivery of the device was performed in view of large ASD and deficient rim.Patient vitals were stable during periprocedural period. Shifted to ICCU after procedure. Post procedure Echo revealed stable device position with no residual and good biventricular function. Post procedure ECG revealed sinus rhythm.Discharged in a stable condition with total duration of 72 hours in hospital.
