Dr. Pai has achieved several notable milestones in cardiac surgery, including:

Dr. Pai successfully excised and reconstructed the infrahepatic IVC with a Dacron graft and implanted the left renal vein into the graft. The patient has been followed up for ten years with no graft thrombosis or tumor recurrence, and maintains normal renal function.
- Closing five defects in the heart in a single surgery.
- Performing reconstructive surgery for bilateral renal artery stenosis.
